How to Send
Apostille and consular authentication applications require original documents only — photos or scans cannot be accepted. Please sign and seal the forms, then mail the originals to the address below.

⚠️ Your seal certificate (Inkan Jeungmyeongseo) must be dated within the last 3 months. An older certificate may be rejected — please check the issue date beforehand.

1. Apostille / Certificate of Authentication Request Form

Official form under the Enforcement Rule on Apostille and Certificate of Authentication issuance [Attached Form No. 1]. Download the original form (PDF) below, then fill it in according to your applicant type (Individual / Sole Proprietor / Corporation) using the sample images as a guide for the highlighted items, and sign and seal it.

⚠️ Please check before printing: Print pages 1 and 2 as the front and back of a single sheet using double-sided printing. In your print settings, choose "Flip on Long Edge." If you print "Flip on Short Edge" instead, the back side will come out upside down.

Individual

Sole Proprietor

You'll need the original seal certificate, a copy of the business registration, and a completed power of attorney. Write the company name and stamp the representative's seal.

Corporation

Fill in the company name (e.g. XYZ Co., Ltd.), business registration number, and company phone/email.

You'll need the original corporate seal certificate, a copy of the business registration, and a completed power of attorney. Write the company name and stamp the corporate seal.

Stamp the company seal along with the corporate seal.

2. Power of Attorney (No Company Seal Stamp)

Official form under the Enforcement Rule on Civil Petition Handling [Attached Form No. 3]. Even without a company seal stamp, the form remains valid as long as the registered seal (corporate seal / representative's seal) is correctly stamped. Individual applicants sign the request form themselves, so no separate power of attorney is needed.

Fill in the company name (e.g. XYZ Co., Ltd.), business registration number, address, and phone number, then write the company name at the bottom signature line and stamp the corporate seal. You'll also need the original corporate seal certificate and a copy of the business registration.

Fill in the company name, business registration number, address, and phone number, then write the company name at the bottom signature line and stamp the representative's seal. You'll also need the original seal certificate and a copy of the business registration.

3. Notarization Power of Attorney (Private Document Certification)

This power of attorney form [Attached Form No. 10-3] is used when an authorized notary office certifies a private document. Fill in the highlighted items according to your applicant type (Individual / Sole Proprietor / Corporation) and stamp your seal.
